The Family Care Community Partnership (FCCP)  is a prevention program funded by the Rhode Island Department of Children, Youth, and Families to support the success of families living in the state of Rhode Island. The FCCP brings community–based service providers, family members, and friends together to build a stronger, brighter future for your family. Whether experiencing issues within the school, home, or community, the FCCP can help your family develop a plan, mobilize natural/formal supports, and assist you as you work toward achieving your goals. The FCCP assists families in navigating services or identifying resources that may be available to help their family.

Wraparound services include:

  • Educational Concerns
  • Housing Insecurity
  • Financial and Employment Concerns/Needs
  • Parental and/or Child Substance Use
  • Parental and/or Child Mental Health, Behavioral, and Developmental Needs
  • Domestic and Community Violence
  • Legal Status / Immigration Resources
  • Youth Diversion Program (YDP) and Wayward Disobedient Petition (WDP)
  • Other factors causing parental and family stress or creating barriers to meeting basic needs such as food, clothing, transportation.
